DISTINGUISHING JOB CHARACTERISTICS

This position involves working under the general supervision of the Youth & Family Program Director. The Case Manager provides direct support to clients through case management services, individual rehab services, and group rehab services. The role is responsible for guiding individuals in setting recovery goals, accessing community resources, and building life skills for long-term stability. The Case Manager will possess skills to maintain a high level of professionalism and ethics when interacting with all clients and community partners.

CASE MANAGER ESSENTIAL DUTIES AND RESPONSIBILITIES

Client Services

  • Ensure that own productivity levels average 55% of billable hours
  • Deliver case management through client linkage, advocacy and referral assistance
  • Collaborate with community providers (substance abuse mental health, prevention, housing, employment, etc.) to support client self-sufficiency and community partnerships
  • Guide clients in defining personal recovery goals and creating action steps to achieve them
  • Model effective coping strategies and self-help techniques
  • Facilitate group and individual rehab treatment services

Intake and Documentation

  • Conduct comprehensive intake screenings for new clients
  • Be primary clinical team member for walk-in screenings
  • Assist in any UAs needed for Treatment Court or MAT clinic
  • Prepare timely, accurate and compliant progress notes
  • Maintain and regularly update a catalogue of current community resources

Program and Team Support

  • Actively participate in treatment team and program development meetings, ensuring inclusion of the client perspective
  • Promote self-directed recovery tools to strengthen client engagement
  • Assist in outreach efforts and represent The Virtue Center and its mission in external meetings

Advocacy and Partnerships

  • Champion the importance of self-sufficiency and community tenure for clients
  • Identify skills present in clients and encourage self-care
  • Serve as community liaison for The Virtue Center as needed in client focused spaces outside of the office
  • Deliver curriculum, prevention and early intervention programs that may take place in the community, local facilities, colleges and schools

OTHER DUTIES AND RESPONSIBILITIES

  • Participates in weekly team meetings and staffings, monthly meetings and in-services as directed
  • Keeps credentials and education current
  • Fill in other staff and programs, taking clients as needed
  • Will work until 7:00 pm up to 3 nights per week and are generally off by noon on Fridays. May need to adjust schedule to meet the needs of the clients and The Virtue Center
  • Completes other duties as assigned by supervisor and Executive Team

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

  • Knowledge of the recovery community and how to access resources in the community
  • Provides experience, education, and professional treatment services to assist and support individuals in recovery
  • Ability to recognize problems beyond the scope of training, skills or competence and refer to team therapists when appropriate

EQUIPMENT OPERATED

Computer, copier, fax machine, general office equipment, cell phone, television, and Microsoft Office Suite.

CONFIDENTIAL INFORMATION/DATA

Client and employee information, charts, documents and records.

CONTACT WITH OTHERS

Clients, family members and significant others, public officials, law enforcement, community professionals, educators/ school systems, courts, attorneys, and the public in general.

QUALIFICATIONS

  • Case Manager II certification with at least 1 year experience in substance abuse, mental health; co-occurring or combination.
  • Peer Recovery Support Specialist Certification is preferred.

LICENSURE OR CERTIFICATION REQUIREMENTS

  • Minimum: Behavioral Health Case Manager II certification (or qualified/in process) and Peer Recovery Support Specialist certification (or qualified/in process).
